What is Case Management?

At JSP, we provide distinct, specialised, client specific services with an overall aim of fully supporting our clients and their families to live as normal a life as possible following potential catastrophic and life changing injuries. We support both adults and children with an approach that holistically involves all family members

Who are our Case Managers and what do they do?  

We have Case Managers who are professionally trained in one of the following backgrounds with a minimum of two years experience, this allows us to introduce specialisms and professional peer support across JSP.

Working with one of our Case Managers will allow our clients complete piece of mind knowing that, throughout what can only be described as a very difficult and traumatic time, we will be there, in their corner, ensuring we encompass professional and personal support at all levels and access to any specialised rehabilitation required to help get the person back to who they were prior to their accident or injury.

Our Case Managers

  • Detailed assessments of your needs, risks, wants and expectations

  • Strategies for minimising the effects of your injury

  • Full recruitment package of support if Support Workers are required

  • Offering you and your family a single point of contact and co-ordination for all your needs

  • A fully supportive back office function to allow our Case Managers to focus on what matters

  • Give you the day-to-day support you need in all aspects of independent living

  • Support and encouragement to achieve your personal goals and expectations

  • Help you to explore possibilities, options and ideas so that you can plan for your future

  • Ensure Support Workers are appropriately trained 

From our initial contact, we will ensure that the client’s safety, welfare and ultimate independence is our main priority and will always ensure the client is as involved as they would like to be…or not….the choice is theirs.
